The health-care economy is filled with unusual and even unique economic relationships. One of the least understood involves the peculiar roles of producer or “provider” and purchaser or “consumer” in the typical doctor-patient relationship. In most sectors of the economy, it is the seller who attempts to attract a potential buyer with various appealing factors of price, quality, and use, and it is the buyer who makes the decision. Such condition, however, is not common in most of the health-care industry.

In the health-care industry, the doctor-patient relationship is the mirror image of the ordinary relationship between producer and consumer. Once an individual has chosen to see a physician — and even then there may be no real choice — it is the physician who usually makes all significant purchasing decisions: whether the patient should return “next Wednesday”, whether X-rays are needed, whether drugs should be prescribed, etc. It is rare that a patient will challenge such professional decisions or raise in advance questions about price, especially when the disease is regarded as serious.

This is particularly significant in relation to hospital care. The physician must certify the need for hospitalization, determine what procedures will be performed, and announce when the patient may be discharged. The patient may be consulted about some of the decisions, but in general it is the doctor's judgments that are final. Little wonder then that in the eye of the hospital it is the physician who is the real “consumer”. As a consequence, the medical staff represents the “power center” in hospital policy and decision-making, not the administration.

Although usually there are in this situation four identifiable participants— the physician, the hospital, the patient, and the payer (generally an insurance carrier or government) — the physician makes the essential decisions for all of them. The hospital becomes an extension of the physician; the payer generally meets most of the bills generated by the physician/hospital, and for the most part the patient plays a passive role. We estimate that about 75-80 percent of health-care choices are determined by physicians, not patients. For this reason, the economy directed at patients or the general is relatively ineffective.

    When seeing the doctor, you feel better to know you will get some medicine. But the doctor knows that medicine is not always needed. Sometimes all a sick person needs is some reassurance (安想) that all will be well. In such cases the doctor may prescribe a placebo.

    A placebo is a sugar pill, a harmless shot, or an empty capsule. The patient thinks it is medicine and begins to get better. How does this happen?

    The study of the placebo opens up new knowledge about the way the human body can heal itself. It is as if there was a doctor in each of us. The doctor will heal the body for us if we let it. But it is not yet known just how the placebo works to heal the body. Some people say it works because the human mind fools itself. These people say that if the mind is fooled into thinking it got medicine, then it will act as if it did, and the body will feel better.

    Placebos don't always work. The success of this treatment seems to rest a lot with the relationship between the patient and the doctor. If the patient has lots of trust in the doctor and if the doctor really wants to help the patient, then the placebo is more likely to work. So in a way, the doctor is the most powerful placebo of all.

    A placebo can also have bad effects. If patients expect a bad reaction to medicine, then they'll also show a bad reaction to the placebo. Some doctors still think that if the placebo can have bad effects it should never be used. They think there is still not enough known about it.

    The strange power of the placebo does seem to suggest that the human mind is stronger than we think it is. There are people who say you can heal your body by using your mind. And the interesting thing is that even people who swear this is not possible have been healed by a placebo.

    Driverless technology will soon enter another major global industry: shipping. Several companies recently announced plans to start self-sailing ships to improve safety and efficiency(效率).

    One of them is Norwegian chemical company Yara International. Yara has designed to build an autonomous ship expected to start next year. The ship is fully electric to reduce air pollution. It is designed to carry containers normally transported by truck. This means the ship will also improve road safety by replacing about 40,000 truck trips a year.

    Yara plans to operate the ships by remote control beginning sometime next year. By 2020, the ships should be loading and sailing themselves. Another company focusing on self-sailing ships is the world's largest mining company, Australia's BHP Billiton. The company's Vice President of Freight, Rashpal Bhatti, recently spoke about the technology. He said, “I believe automation(自动化)will be one of the biggest changes for shipping in the future.” Autonomous ships offer important chances to improve safety and provide better efficiency results to the marine supply chain. He added that self-sailing ships could become a reality within the next decade.

    Rolls-Royce, the British automobile and engine manufacturer, is also developing unmanned shipping technologies. Oskar Levander is the company's head of marine operations. He told a conference last year, “This is happening. It's not if, it's when.” Several shipping companies have invested hundreds of millions of dollars to develop self-sailing ships expected to be used by 2025. These ships will be equipped with technology to collect weather and sea data. They will then use the data to plan out the safest, shortest and most fuel-efficient route.

    The idea of turning recycled plastic bottles into clothing is not new. During the last five years, a large number of clothing companies, businesses and environmental organizations have started turning plastics into fabric to deal with plastic pollution. But there's a problem with this method. Research now shows that microfibers could be the biggest source of plastic in the sea.

    Dr. Mark Browne in Santa Barbara, California, has been studying plastic pollution and microfibers for 10 years now. He explains that every time synthetic (合成的)clothes go into a washing machine, a large number of plastic fibers fall off. Most washing machines can't collect these microfibers. So every time the water gets out of a washing machine, microfibers are entering the sewers and finally end up in the sea.

    In 2011, Browne wrote a paper stating that a single piece of synthetic clothing can produce more than 1, 900 fibers per wash. Browne collected samples from seawater and freshwater sites around the world, and used a special way to examine each sample. He discovered that every single water sample contained microfibers.

    This is bad news for a number of reasons. Plastic can cause harm to sea life when eaten. Studies have also shown that plastic can absorb other pollutants.

    Based on this evidence, it may seem surprising that companies and organizations have chosen to turn plastic waste into clothing as an environmental "solution." Even though the science has been around for a while, Browne explains that he's had a difficult time getting companies to listen. When he asked well-known clothing companies to support Benign by Design—his research project that seeks to get clothes that have a bad effect on humans and the environment out of the market, Browne didn't get a satisfying answer. Only one women's clothing company, Eileen Fisher, offered Browne funding.

    The "Give It Up for Earth Day" encourages people to commit to giving up actions that are harmful to the environment, such as using poisonous cleaners.

    Throughout the month of April, Canadians are being asked to "Give It Up for Earth Day!" Earth Day has been celebrated every year on April 22 since 1970. Earth Day Canada president Jed Goldberg says that as people become more environmentally aware, they want to find ways to reduce their environmental influence, not just celebrate one special event.

    "Earth Day is a great launching pad for thinking about environmental action every day," said Goldberg. "Choosing healthier options, even for the short term, can lead to thinking about the influence of our decisions for a lifetime."

    That's the reason behind the "Give It Up for Earth Day" campaign. It's designed to encourage healthy habits that benefit people and the planet. It challenges everyone to help create a healthier world by making changes in their daily routine.

    "We wanted to plan an action—targeted campaign that will give people a chance to act on their concern for the environment in a positive way," said Goldberg.

    The campaign has identified four daily actions that can add up to huge environmental savings. They include pledging (保证) to turn off the TV, give up the use of poisonous cleaners, reduce consumption of consumer products and cut out meat.

    At the end of April, all of the pledges will be recorded and converted (转化) into the number of TV hours not watched, dollars not spent on unnecessary items, electricity saved, greenhouse gas emissions (排放) avoided by cutting out meat, and poisonous chemicals produced from homes and landfills.

    "What we're trying to achieve this year for Earth Day is to give people a chance to do something that is meaningful and measurable," said Goldberg.

Max Du emerged victorious at the Canada-Wide Science Fair with his innovative project: an unmanned aerial vehicle (UAV) designed to assist individuals experiencing cardiac arrests. His ingenuity was sparked during the Christmas recess of the preceding year. "I received a recreational drone as a gift from my parents, yet I was unable to operate it aloft due to the snowy conditions," Max recounted. "Consequently, I experimented with it indoors, which led me to contemplate the potential for a drone to serve as an automated indoor apparatus that could render assistance to people in need."

Approximately 35,000 Canadians succumb to cardiac arrests annually, with the majority of these incidents occurring beyond the confines of a hospital, resulting in a survival rate of less than 10 percent. Max was convinced that a drone could provide more expeditious assistance and expedite the delivery of vital medical treatments, but to validate this hypothesis, he was compelled to construct the device himself.

The process of refining his drone spanned a period of six months, during which Max's parents had to tolerate the persistent buzzing and crashing of the machine within their domicile. With each innovative feature he devised, such as a telescopic appendage, the drone's overall mass increased, leading to frequent disassembly. This necessitated the procurement of an entirely new set of components. Max persevered, experimenting with more lightweight construction materials until he achieved a more harmonious equilibrium in his design.

Through a methodical process of iterative refinement, the 14-year-old prodigy ultimately perfected his invention. His drone is now capable of manipulating a door handle, navigating through the air, and alighting gently on the ground. An innovative, extendable arm can be deployed to either administer an injection or deliver a life-saving pharmaceutical to a patient. Additionally, an integrated camera facilitates real-time communication with an emergency response team, enabling remote monitoring of the patient's condition.

Max is intent on pursuing the acquisition of a patent to forge alliances within the healthcare sector and bring his invention to fruition. He dedicated his summer to the study of artificial intelligence at Stanford University in California, where he was one of only 32 young scholars chosen globally. Subsequently, he will proceed to the University of Pennsylvania to enroll in a collegiate-level robotics course before resuming his high school studies in September.
