Unhealthy health care bills, long emergency-room waits and inability to find a primary care physician just scratch the surface of the problems that patients face daily.

Primary care should be the backbone of any health care system. Countries with appropriate primary care resources score highly when it comes to health outcomes and costs. The U.S. takes the opposite approach by emphasizing the specialists rather than the primary care physician.

A recent study analyzed the providers who treat Medicare Beneficiaries(老年医保受惠人). The startling finding was that the average Medicare patient saw a total of seven doctors — two primary care physicians and five specialists — in a given year. Contrary to popular belief, the more physicians taking care of you don't guarantee better care. Actually increasing breakup of care results in a corresponding rise in costs and medical errors.

How did we let primary care slip so far? The key is how doctors are paid. Most physicians are paid whenever they perform a medical service. The more a physician does, regardless of quality or outcome, the better he's reimbursed (返还费用). Moreover, the amount of a physician receives leans heavily toward medical or surgical procedures. A specialist who performs a procedure in a 30-minute visit can be paid three times more than a primary care physician using that same 30 minutes to discuss a patient's disease. Combine this fact with annual government threats to indiscriminately cut reimbursements, physicians are faced with no choice but to increase quantity to boost income.

Primary care physicians who refuse to compromise quality are either driven out of business or to cash-only practices, further contributing to the decline of primary care.

Medical students are not blind to this scenario. They see how heavily the reimbursement deck is stacked against primary care. The recent numbers show that since 1997, newly graduated U.S. medical students who choose primary care as a career have declined by 50%. This trend results in emergency rooms being overwhelmed with patients without regular doctors.

How do we fix this problem?

It starts with reforming the physician reimbursement system. Remove the pressure for primary care physicians to squeeze in more patients per hour, and reward them for optimally managing their diseases and practicing evidence-based medicine. Make primary care more attractive food to medical students by forgiving students loans for those who choose primary care as a career and harmonizing the marked difference between specialist and primary care physician salaries.

We're at the point where primary care is needed more than ever. Within a few years, the first wave of 76 million Baby Boomers will become qualified for Medicare. Patients older than 85, who need chronic care most, will rise by 50% this decade.

Worlds of Fun

    Location: Worlds of Fun is located off Highway 435 in Kansas City, Missouri.

    History: Worlds of Fun was opened on May 26, 1973, at a cost of 25 million dollars. Loosely themed around the Jules Verne book, Around the World in Eighty Days, the park was founded by Hunt Midwest Company. In 1982, Hunt Midwest bought a nearby waterpark, Oceans of Fun. In 2013, Worlds of Fun and Oceans of Fun were combined to a one ticket admission, providing all guests with access to 235 acres of amusement and water rides.

    Hours: Worlds of Fun is open from April through Halloween.

    Tickets: Buy and print online. Always try to buy your tickets in advance, to save time when you get to the park.

    Reservations: Worlds of Fun sells “Fast Lane” cards that save riders' time by allowing them to bypass the majority of wait for most of rides and attractions including Mamba, Prowler, and Patriot. Ride as many times as you want all day long.

    Strategy: Most visitors tend to begin the day with Prowler, the hottest attraction in the park. Use that tendency to your advantage and head to the Patriot first. After that, try the Dragons. Then work your way back to the Prowler. After riding the Prowler, there is only one coaster left, Mamba. Hit it next. If the park is not very crowded, you can ride Boomerang on the way to Mamba. After riding Mamba, head back for a ride on the Wolf. By then you will have tried most of the popular rides and attractions in the shortest possible time.

    News: In 2017, Worlds of Fun is adding Steel Hawk, a ride that will take guests up 301 feet in the air and spin them at a 45-degree angle for a 60-second flight. Wait to have a try!

    Half a century ago, Japan built the world's first high-speed rail network—a network that remains the gold standard in train travel today. Currently the country is now helping Texas build its own bullet train, a potential game-changer for transportation in the state.

    When it launched on October 1, 1964, the world's first high-speed rail network was known as the “super-express of dreams.” The first line in Japan's now world-famous shinkansen network was built against all odds, in the face of fierce public opposition, technical difficulties and astronomical costs.

    Half a century ago, the system was far humbler. In 1964, the first track was a 320-mile-long link between Tokyo and Osaka that reduced the trip from six-and-a-half hours (on conventional trains) to three hours and 10 minutes, traveling at a maximum speed of 200 miles per hour. For the first time, workers could get to meetings in one city during the day and be back home drinking a beer in the local pub that night.

    Not only did the train expand mobility profoundly, but also businesses appeared around the major stops as a growing emphasis on productivity swept across Japan. Today, the shinkansen network has 1,487 miles of track, with more set to open in the coming years. It seems that everything the shinkansen touches turns to city, and regions that are off the beaten track, so to speak, benefit greatly from the economic jumpstart brought by the train. New shinkansen lines are often proceeded by aggressive marketing campaigns promoting tourism in those areas, a strategy that seems to work.

    Despite its astronomical costs, it actually has saved more. Today, over 350,000 annual trips transport tens of millions of passengers all over Japan with efficiency—the average delay time is less than a minute. A research report titled 30 Years of High-Speed Railways: Features and Economic and Social Effects of The Shinkansen by Hiroshi Okada, estimates that the economic impact from the shinkansen train network, based on the time saved from faster travel, is approximately ¥500 billion ($4.8 billion USD) per year. Okada stresses that the cultural impact is also significant, a shinkansen offers people living far from urban centers “easy access to concerts, exhibitions, theaters, etc., enabling them to lead fuller lives.”

    Japan has a plan, known as the One-Day Travel Initiative. Its goal: regardless of where you are in Japan, it should only take you three hours to get to the nearest major regional city (Tokyo, Osaka, Nagoya, Sapporo or Fukuoka). The planned impact of this hyper-mobility is to discourage the tide of migration toward urban centers, like Tokyo, and encourage decentralization.

    American and British people use different greetings. In the USA the commonest greeting is “Hi”. In Britain it is “Hello!” or “How are you?”. “Hi!” is creeping(不知不觉地进入) into British, too. Many verbs in American English with “ize” or “izing” forms, e.g. organize, realizing are spelt in BrE with “ise” or “ising”, e.g. organise, realising. In American English, “practice” is used both for the verb and noun. In BrE, the verb is spelt “practise”, and the noun “practice.” In American English, one writes “traveler,” while in British English, one writes  “traveller”.

    It was once predicted that British and American English would become separate languages finally. But the opposite has happened. The links(联系) between the two countries are so strong that linguistically(语言上地),and probably culturally(文化上地) too, they are closer together than ever.

    Christmas is a time for eating great food, giving and receiving presents, and most importantly spending time with your family watching some classic Christmas movies.

The Grinch (2000)

    This is a Christmas must-watch and one of Jim Carrey's best performances. It was also originally a nursery tale book written by Dr Seuss. It's the story of a green monster who wants to ruin Christmas but who in the end discovers the power of love and generosity. I promise it's a great film with plenty of laughs along the way.

    Love Actually (2003)

    While this is not a personal favourite of mine, people in the UK absolutely love this film. It has many featuring actors and actresses. It might be a little difficult to follow as there are complex plots, so I recommend watching a translated version or enabling subtitles (字幕) in your native language. However, the heart-warming elements of the film make it worth watching.

    Home Alone (1990)

    This amusing masterwork is one of my favourite films of all time. It tells the story of a boy, Kevin, who is left alone in his house when his large family forgets to take him on vacation with them. While the families are away, burglars try to rob the house and Kevin must protect his home by setting traps. The trips, falls and traps will have you laughing out loud as you watch a 10-year-old boy beat two grown men.

    Miracle on 34th Street (1994)

    You can't have Christmas in an English-speaking country without watching this movie. It's not funny like Home Alone or The Grinch, but it is incredibly touching. It's about a man who claims to be Santa, but nobody believes him except a lawyer and a little girl. The story-line may seem a little childish but you can enjoy it at any age.

    John Blanchard was studying the crowd making their way through the station. He was looking for the girl whose heart he knew, but whose face he didn't, the girl with the rose.

    When reading a book in a Florida library a year before, John became interested not in the contents of the book, but in the notes penciled in the margin (空白处). The handwriting reflected a thoughtful soul and beautiful mind. He discovered the former owner's name in the front of the book: Miss Hollis Maynell.

    He located her address and wrote a letter introducing himself. The next day he was shipped overseas to serve in the army. During the next year, they grew to know each other through the mail and their friendship developed. John requested a photograph, but she refused, saying if he really cared, it wouldn't matter what she looked like. When the day finally came for him to return home, their first meeting was suggested-7:00 pm at the Grand Central Station in New York.

    She wrote, "You'll recognize me by the red rose I wear on my coat." So now John was in the station to meet the girl with a rose.

    As a pretty and slim girl in green came over, John noticed her blue eyes like flowers in spring. He walked directly towards her, entirely forgetting she was not wearing a rose. As John came closer to her, he saw another woman with a red rose stood nearby. Well past 40, this woman had graying hair done under a worn hat. Seeing the girl in green walk quickly away, John felt as if he were split (劈开) in two. He desired to follow that girl, but longed for the woman whose spirit had truly companioned and supported him.

    The woman looked gentle and sensible. John went to her, saying, "I'm Captain John Blanchard. You must be Miss Maynell. I am glad to meet you here. May I take you to dinner?"

    She replied with a smile, "I don't know what this is about. But the lady in green who just went by, begged me to wear this rose on my coat. She said if you asked me out to dinner, I'd tell you she is waiting for you in the big restaurant across the street. She said it was some kind of test!"
